Coming home after a stay in hospital is often a relief, but it can also feel overwhelming. At Caremark York & Selby, we provide friendly, professional home care that helps people recover safely in familiar surroundings. Personalised support when supporting someone after a hospital stay in York and Selby

Everyone’s recovery is different. That’s why we take the time to understand your hospital discharge notes, listen to what matters most to you and develop a care plan that supports your recovery from day one.

Our Care Assistants can help with medication reminders and collection, support with washing and dressing, safe movement around your home and preparing nutritious meals while encouraging regular hydration. In addition, where required, we also work alongside district nurses and other healthcare teams to help make sure wound care continues as planned.

As your recovery progresses, we adjust the length and frequency of visits. Therefore, you can gradually become more independent while knowing support is there whenever you need it.

What is reablement care?

Reablement care is short-term support that helps people regain confidence and independence after illness, injury or surgery. Rather than doing everything for you, our Care Assistants encourage and support you to rebuild everyday skills at a pace that feels comfortable.

This might include practising safe transfers, managing stairs, regaining skills to carry out daily activities, such as cooking, improving mobility around the home or gradually returning to daily routines. As your confidence grows, we reduce the level of support so you can continue doing more for yourself.

Local expertise you can rely on

As a local provider, we know York and Selby well, including Clifton, Acomb, Poppleton, Copmanthorpe, Haxby and Selby. That local knowledge helps us provide reliable visit times and respond quickly if circumstances change.

All of our Care Assistants are employed by Caremark, fully trained and DBS-checked. Their training includes falls prevention, safe moving and handling, infection control and person-centred care. Wherever possible, we also take care to match customers with Care Assistants who suit their needs and personality. As a result, this helps to build trusted relationships from the very beginning.

Getting started

We’ll begin with a friendly conversation and a free home assessment to understand your circumstances, discuss your goals and recommend the right level of support.

We can also talk you through the different ways care after leaving hospital can be arranged and funded, including support from your local authority. This may include a care needs assessment, personal budgets or direct payments, depending on your situation. You may choose to arrange care yourself or have support from the council in organising services that meet your needs.

Once your care begins, we’ll make sure medications, equipment and daily routines are in place. We also carry out regular wellbeing checks. Your care plan will be reviewed with you, your family and any healthcare professionals involved. In this way, it continues to meet your changing needs.
