  1. Firstly we complete comprehensive medication assessments. We conduct thorough assessments of each client’s medication regimen, including prescription medications, over-the-counter drugs, supplements, and herbal remedies. This assessment considers factors such as dosage, frequency, potential interactions, and individual preferences.
  2. We also create individual care plans. Based on the assessment, we develop customised care plans that outline medication administration management tailored to each client’s unique needs and preferences. These plans are regularly reviewed and updated to reflect changes in the client’s health status or medication regimen.
  3. We also use medication management tools. As well as blister packs we use technology like our Care Planner App. This lives on your carers phone and details the customers care plan including what medication to take and when. It also allows our office to see in real-time that medication has been given or if, for example a customer has refused to take the medication. Based on this we can assess if family members or medical professionals need to be advised.
  4. We give our team comprehensive training and education: They undergo rigorous and regular training and education on medication administration best practices, including medication safety policies and procedures, and techniques for administering various forms of medication.
  5. Then we have supervision and oversight: Our Carers are given regular supervisions from our Care Supervisor to observe and provide continuous training and guidance.
  6. Our team also collaborates very closely with other Healthcare professionals. We maintain open lines of communication with doctors, pharmacists, and specialists. This collaborative approach ensures coordination of care, timely updates on medication changes, and adherence to treatment plans.
  7. We encourage client and family engagement: We actively involve clients and their families in medication management processes, encouraging open communication, asking questions, and addressing any concerns or preferences. This collaborative approach fosters transparency, trust, and empowerment.
