Could you lead and motivate a team of Support Workers to enable our customers to lead a happy and ordinary life?
Our service supports Children, Young People & Adults who live in their own home environments who have health conditions such as Autism, Cerebral Palsy and Downs Syndrome as well as helping with PEG feeds and tracheostomy cares where required.
Our Case Leader role is community based, as such, it is important that you are organised, have strong planning skills and are able to use your own initiative. You will need to be highly visible in the community, visiting your assigned customers on a regular basis and being a central part of the service that we provide. As the role is community based, we require candidates to hold a full UK driving license and their own vehicle.
Our organization enables our customers to live a happy lifestyle and do things that any other individual would, a few examples are:
- Our support workers help with sensory activities, including baking buns and cakes, playing with slime, play dough and other sensory toys- our Support workers have great fun!!
- They go to the Seaside and have fun on the beach (when the weather allows!)
- We attend schools/colleges with our customer and get involved in 1:1 activities such as painting, drawing, and group activities with other children.
- Our carers will go and play football in the park or go for muddy walks, farms. (which we all secretly love!)
- We support our customers to do fun things like bowling, swimming, trampolining or playing football.
- Helping them learn how to do things for themselves like reading, writing, counting, making their own lunch, brushing their own teeth.
- Some of our customers have complex health conditions, our Registered Nurse trains our staff on how to support every individual to achieve their full potential
As a Case Leader you will be monitoring the service and making sure all our Support Workers are helping our customers live their best life!
As well as all the FUN things!!! – Our Case Leaders also do the following:
- Monitor, support and develop a dedicated staff team, inspiring them to be focused on achieving their customer’s goals and outcomes. (The fun stuff listed above!)
- Keep in close contact with our customers, family members and professionals.
- Supervise an allocated number of customers and staff – great communication is essential in any service, ours is especially important!
- Conduct regular staff spot checks, reviews and observations of care practice- we want our staff to be the very best that they can be in order that they can achieve their full potential!
- Work flexibly and plan their own diary in order to meet the requirements of the service whilst also maintaining a good work-life balance – it is really important to us that our team ENJOY their own family time!

What we expect from you:
- We need you to be full of energy and POSITIVE!
- We want you to really be involved with your customers care visits and genuinely care about their customer’s needs and wishes.
- Be able to plan and respond to our customers needs quickly and efficiently.
- Be flexible in your work pattern including ‘On Call’ Evening & Weekend support (on a rota basis, 1 in 3 weeks)
- Great Communication – needs to be fantastic!
- CARE about your team, listen to them, guide them and support them and make sure they feel valued.
Salary:
£22,000 – £24,000- pa dependent on experience.
28 days holiday (Incl Bank Holidays)
