Administrator/Care Planner
Caremark Rotherham & Sheffield are seeking a dedicated and compassionate Care Coordinator to join our experienced management team. We proudly deliver high-quality support to children and adults living with complex needs in the community and are rated ‘Good’ by the CQC.
About the Role
As a Administrator/Care planner, you’ll play a key role in ensuring that our clients receive consistent, personalised high-quality care. This is a dynamic position requiring excellent organisation, problem-solving, and communication skills within a fast-paced environment.
Key Responsibilities
- Effectively coordinate and maintain Support Workers’ rotas, ensuring continuity of care and appropriate client-staff matching.
- Allocate fully trained staff in line with client-specific care requirements.
- Respond efficiently to telephone and email enquiries.
- Use care planning and rostering systems such as People Planner and Access Care Planning (ACP).
- Liaise with clients, families, healthcare professionals, and colleagues to ensure seamless service delivery.
- Respond to changes in care requirements or scheduling swiftly and professionally.
- Provide regular support and supervision to the care team.
- Participate in the on-call rota as part of a shared team responsibility.
- Hold a valid UK driving license, access to a vehicle, and the ability to obtain business insurance.
Key Requirements
- Previous experience in a care coordination or similar administrative role within health or social care.
- Strong IT skills and proficiency in Outlook, Word, and Excel.
- Ability to prioritise tasks and manage competing demands under pressure.
- Excellent communication and interpersonal skills.
- A strong commitment to person-centred care and high standards of service.
