Risk of falls

Mary* was referred to Caremark following a number of falls which left her with a broken arm. At further risk of falls, her independence at home was at risk so we put a care plan in place to support her whilst she recovered from her injury.

Our team carried out tasks she was unable to do, whilst helping her do jobs which she was able to perform.

Over time, with our support, she slowly regained her full independence, at which stage our support ceased.
