Care at home after Hospital Discharge

After hospital care is an in-home service provided by a qualified professional Care & Support worker who will assist you with the process of returning to your home following formal discharge from hospital.

Having a robust support plan in place for transitioning home is an effective way to manage post-hospital recovery risk

As one of our main services, our Care & Support workers will work directly with your doctors and other healthcare professionals involved in your treatment. They will ensure you’re not only ready to return to your normal daily routine in the comfort of your own home, but will also actively assist you with everyday tasks – such as cleaning, cooking, grocery shopping and attending appointments.

Help and companionship as you recover

Everyone is different, so your after hospital care plan will be unique and tailored to meet the challenges of your specific health conditions, recovery requirements, daily habits and lifestyle goals. Our Care & Support workers are also selected with your direct input, to match your personality.

Generally speaking, after hospital care is divided into two main areas. The first focus is assisting you with those day to day tasks you may need a little extra help completing, as well as getting you back on track to help manage them yourself as much as possible. The second is focused on your overall wellbeing as a person.

Services we can arrange for you

Care at home  following hospitalization is incredibly varied and should be highly personalised to suit your needs. We can help you find the right service to meet your needs. Below we share some of the services we can offer you. You may need one or several services and we can discuss this with you.

Escort you home

A carer can be with you from the very moment you are discharged from hospital. They can help you with your belongings, transport you safely home and then once home they can help to settle you in.

Help with discharge instructions

Every patient is given discharge instructions and information when leaving hospital. This can be a lot of information to follow and remember. Your dedicated carer can help to remind you about your post hospital care.

Follow up appointments

In many cases you will need some form of follow up care at the hospital. This may be for a routine x-ray or to see your specialist. Your carer can go with you to this appointment and offer much needed moral support.

Prescription and medication

A carer can help to remind you when you need to take your medication. They can also collect any medication that you may need from the pharmacy and help you with repeat prescriptions.

Personal care

You may need a little extra help with personal care needs once you return from hospital. This is especially true if you have had surgery or if you need an extended period of rehabilitation. Live in carers can assist you with all personal care requirements.

Shopping

You may still feel a little weak or have reduced mobility following your hospital stay. A carer can do your shopping and unpack it all for you as well. If preferred they can help you to organise an online delivery.

Preparing meals

It is important that you eat regular healthy meals to aid your recovery. A live in carer can help you to prepare meals and ensure that you are eating a well-balanced diet.

Housekeeping

If you are worried about doing the household chores once you return home from hospital and feel that you would benefit from a little help, then a live in carer can assist you with cleaning and laundry.

Post-operative exercises

You may be given post op exercises to do at home to aid your recovery. A live in carer can assist you with these exercises to help you build up your mobility and stamina.

Benefits of Post Hospital Care

There are many benefits of using post-hospitalisation care services.

Reduce risk of readmission

One of the biggest benefits of using a care service is that the risk of readmission is greatly reduced, especially for those who are elderly or have dementia. This is because they will be well supported at home.

Personalised service

The care service can be highly personalised to suit you, your recovery plan and the environment in which you live. You will be fully assessed to see what type of care is needed.

Quicker recovery

When you have care at home then you will make a quicker recovery. This is because your stress levels will be greatly reduced and you will have help in getting around your home, reducing the risk of falls and injury which could hinder the recovery process.

Good for your mental health

The carer will provide you with a great amount of psychological support. You will be able to share any worries or concerns you may have with them, and this is essential for good mental health.

How long can I receive after hospital care?

Once again it all depends on your specific circumstances and personal preferences. Some of our customers require daily, overnight, or 24 hour care for several months following their return home. Others only require intensive assistance for the first week following discharge, and just a few hours per week afterwards, and may only need minimal help with cleaning or shopping for a short time.

The important thing to know is the care is available as long as you need it to get back on your feet again and remain in your own home!

We also provide palliative care for customers with a terminal illness who’ve been discharged from hospital and who wish to live their remaining days in the comfort of their own home. This is achieved through thoughtful help and support.
