Caremark is a leading Registered Provider of Nurse led Care & Support to Children, Young People & Adults living with Complex Health conditions in the Sheffield and Rotherham area. With over 10 years experience working in the Complex & Clinical Health Care sector enabling our customers to live an ordinary fulfilled life in their own homes and communities.
We are proud to engage with Independent Case Management Organisations, providing bespoke support to their Clients, Case Managers and associated therapy and legal teams for both post and pre settled cases. Collaboration with Case Managers on cost effective care planning, initial assessments, and real time factual, evidenced based disclosable documentation.
Our service is Nurse led with proven pathways of delivery rated Good by the CQC. Providing high quality, tailored health care solutions, practical and emotional support for customers, through consistent, regular assigned staff teams. We develop bespoke support packages for everyday life skills, vocational activities, extending to within School, College workplace environments and holiday breaks.

Alice
Alice is 6 years old and was pre-trial when Caremark commenced supporting her. Alice has quadriplegic cerebral palsy, following an injury at birth due to clinical negligence. She is, visually impaired and requires suctioning, oxygen and receives her feeds by PEG. Alice required 2 to 1 support 24 hours per day. Following initial assessment and engagement with her Case Manager, we developed a specifically trained team of Support Workers who’s experience and skills matched those required for Alice. We also considered her aspirations, personality and family dynamic. We developed a consistent robust schedule of support encompassing 12-hour shifts. We delivered support for
Alice and her family overnight and throughout the day. Working alongside therapists we developed a rehabilitation plan including hydrotherapy sessions. We enabled Alice to access her community by providing staff who could drive her mobility vehicle. She was supported to attend school including after-school activities and create goals such as family holidays which we supported to plan
and achieve.

Paul
Paul is a 30-year-old adult, he was left with a brain injury- following an unprovoked attack. His request for support came to Caremark as a post-trial package through his Case Manager. Paul lost confidence in doing everyday activities and had physical restrictions. He required support to:
- Maintain his home environment
- Do housework
- Do shopping meal preparation
- Access his community and safely attend appointments.
He also required support with his Oxygen Management. We introduced experienced support workers who we felt had the relevant experience and people skills that matched Pauls’s interests. Paul required support three times a week for 5 hours.
We were able to respond to his package rapidly and provide a team who are consistent, which supports the development of Paul’s confidence and independence. He enjoys having regular support workers who he has become to have trust in and know well. This has
enabled him to recommence his interest in Wood Turning which he now aspires to turn into a small business.

I feel Caremark work really well with the Childrens Continuing Care team. We have a good level of communication and I value the regular catch up meetings which allows us to work together to achieve the outcomes for the children and young people. I find Caremark incredibly flexible in their services and their ability to adjust to packages at short notice. I also appreciate their honesty if they don’t think the package is suitable for them to take on. Given the nationwide problems with recruitment, Caremark have been consistent in their ability to provide
suitable staff to deliver packages throughout.
