To help reduce unnecessary hospital admissions, the early intervention vehicle aims to see housebound patients at home within two hours. An Urgent Care Practitioner and a member of the social team crew every vehicle.
What they can do:
- Clinical assessment with medications
- care for falls without injury or minor wounds
- Support with poor mobility (Stuck in chair or bed) and mobility aids
- Support for carers/family who are struggling to cope
- Emergency care packages
- Onward referral to other prevention of admission teams.
