End of Life Care

Choosing where to spend your final months is a deeply personal decision, and if home is where your heart is, we’re here to make that possible. Our commitment is to provide you and your loved ones with the support and care you need during this time.

We take the responsibility of respecting, protecting, and promoting your dignity very seriously. This means involving you in every step of planning and delivering your care. We recognize the importance of including those close to you in this process, and with your consent, we’ll collaborate closely with anyone you choose to involve.

Our goal is to ensure that our service not only meets your needs but also provides comfort in knowing your loved ones are participating in your care as you wish. We’re here to offer support not just to you, but also to the important people in your life who stand by you.

NHS Continuing Care Funding

You might be eligible for NHS funding to help with your care costs. To access NHS Continuing Care Funding, you need to be assessed as having a ‘primary health need.’ If you qualify, there’s no means test involved. You can find more details in the article linked here.

Don’t forget to also check out the NHS website for additional information. Another fantastic resource is the Care To Be Different website, especially if you’ve faced challenges with your funding application.

And remember, if you do receive NHS Continuing Care funding, you always have the freedom to choose your own care provider.

Putting Your Affairs in Order

We work with a number of people and organisations who can help you put your affairs in order. Some of the more important things to consider are:

Ensuring you have a will in place.

Ensuring you have appointed a person(s) to act as your power of attorney.

Your financial affairs.

Although these are not our areas of expertise, we can signpost you to those who do have expertise in these areas.
